The original cover for Scorpions' Virgin Killer album would probably be the winner, since it could qualify as child pornography (there was even a legal court case in the UK over the image being on Wikipedia) and would almost certainly result in a ban were it shared here, hence why I'm not posting it. Other candidates: Note: the Gentle Giant cover is of a peach, not an ass, still a hideous cover though.
